    1. Must be miscarriage, stillbirth, or death of an infant up to one year of age
    2. Must be resident of one of the following LA parishes:
    o Lafayette
    o Vermillion
    o Iberia
    o St. Landry
    o Acadia
    o Avoyelles
    o Evangeline
    o Jeff Davis
    o St. Martin
    o St. Mary
